Patient Specific Education ONC Criteria: 170.315(a)(13) This functionality enables user to identify patient‐specific education resources based on data included in the patient's problem list, medication list, and laboratory tests and values/results using infobutton standards. Social, psychological, and behavioral data ONC Criteria: 170.315(a)(15)Care PlanONC Criteria:170.315(b)(9) This functionalities are available as an add-on module for CPC+ and allows user to capture the patient’s Social, psychological and behavioral data, document Care Plan and sharing of care plan with the Care team members. This is an add-on module to the EHR available for additional one-time setup cost and recurring monthly support charges per provider. No contractual limitations No technical or practical limitations
Transition of Care ONC Criteria: 170.315(b)(1)-(2) This functionality allows the EHR user to receive, display, incorporate, create and transmit a Transition of care/referral summaries or Discharge Summary. Additional one-time setup and annual recurring cost for each direct address purchased. There is no limit for the number of direct messages sent/received. Direct Address needs to be purchased for each provider and clinical user who wish to send and receive direct messages. iPatientCare HISP works with only those HISPs which are part of the Direct Trust Bundle. iPatientCare implements and is certified using the IHE XDR edge protocol and can communicate with any other third-party HISP using the IHE XDR Direct Edge Protocols.
e‐Prescribe ONC Criteria: 170.315(b)(3) This functionality enables users to electronically create prescriptionsand electronic transmission of prescriptions including prescriptions for controlled substance. Optional one-time and yearly subscription fees are required for EPCS and interfaces with any third-party vendor and/or service provider. Electronic prescriptions may only be transmitted to members of Surescripts network, which requires the execution of the SureScripts End User License Agreement. Requires the use of Surescripts Network.
Data Export ONC Criteria: 170.315(b)(6) This functionality enables users to electronically create a set of export summaries for one or more patients. No additional costs except pass-through charges to be paid to third-party entities, such as, the data center and outsourced data extraction agencies, whichever applicable No contractual limitations Exporting data for all patients or many patients is a resource intensive task and may slow down the EHR (especially in Cloud Hosted environment). Hence it is recommend to schedule this during off hours.
Clinical Quality Measures ONC Criteria:170.315(c)(1-4) This functionality enable users of EHR to capture, import, calculate and report on electronic clinical quality measures. No Additional costs. This is covered in the monthly support/subscription costs. This feature is included in the EHR and there are no contractual limitations as far as the use of these tools are concerned. iPatientCare provides value added Quality Improvement Consulting services to assist the providers in improving the quality of care, resulting in better performance under Quality Payment Program. These services are not bundled in the EHR and need separate contract because not all providers need these value-added services.If custom data extractions are needed for any 3rd party application, appropriate contracts need to be in place between iPatientCare and the 3rd Party application. API Access ONC Criteria:170.315(g)(7)-(9) This functionality allows the patient to access their health record though any third party application that has built interface with iPatientCare FHIR APIs Any costs or fees associated with the use or implementation of any third-party applications is outside the scope of iPatientCare software licensing.Any iPatientCare engagement related to the integration of a third-party application with the EHR may involve additional fees. Application Developers who would like to provide access to patient health information through the FHIR API need to agree to the Developer Terms of Use and FHIR API License Agreement associated with establishing a Developer Open Access Account.iPatientCare does not provide or recommend any third party application that uses iPatientCare FHIR API. Patients (or Providers) are responsible to identify and purchase applications from developers who have signed the developer agreements with iPatientCare. Unreasonable resource utilization due to un-optimized implementation of the APIs by the third-party applications may cause performance issues which are beyond the control of iPatientCare.
