This functionality provides evidence- based clinical decision support along with diagnostic and therapeutic reference information based on patient’s demographics, problems, allergies, medications, Lab test and vital signs using HL7 Version 3 Standard: Context Aware Knowledge Retrieval Application. (“Infobutton”), Knowledge Request, Release 2 standardsiPatientCare provides MedlinePlus as the source of diagnostic and therapeutic reference information, which is free of cost. If a provider wishes to use any other reference information sources, a separate subscription with the content provider may be required.

This functionalities are available as an add-on module for CPC+ and allows user to capture the patient’s Social, psychological and behavioral data, document Care Plan and sharing of care plan with the Care team members.This is an add-on module to the EHR available for additional one-time setup cost and recurring monthly support charges per provider.
Transition of Care
