MACRA has made way for a new framework of the Quality Payment Program (QPP) for rewarding the health care providers for giving better care. Under QPP there are two pathways – Merit-based incentive payment system (MIPS) and Advanced Alternative Payment Model (APM). Comprehensive Primary Care Plus is one such Advanced APM. It offers greater rewards for taking some risks related to patient outcomes.

CPC+ is a national advanced primary care medical home model that aims to strengthen primary care through regionally-based multi-payer payment reform and care delivery transformation. It consists of the three payment elements – care management fee, performance-based incentive payment, and payment under the Medicare physician fee schedule. This supports the delivery of comprehensive primary care.
